Nurse Practitioner

This role offers Nurse Practitioners the rare opportunity to build meaningful relationships during some of the most emotionally complex moments in a person’s life. You’ll walk with patients and families through uncertainty—holding space for grief, hope, planning, and honest conversations. With time to listen and the trust to guide, you’ll help shape care that honors each person’s experience, values, and needs.
Nurse Practitioner (NP) Qualifications:
- 1+ years of Nurse Practitioner experience in an acute care environment.
- Master of Science in Nursing - Family Nurse Practitioner.
- Active, unencumbered state NP license.
- Home Health, Palliative Care, or Hospice Experience preferred.
- Current driver's license and auto liability coverage.
- Detailed assessment skills in this role require recertification for Hospice patients.
- Interest in a community-based role that requires travel between patient visits.
- Drug Enforcement Agency (DEA) number required.
- National Provider Identifier (NPI) Required.
Nurse Practitioner (NP) Schedule & Benefits:
- Schedule: Monday to Friday, 8:30 AM - 5:00 PM with no on-call required
